Fourth Grade Pool Party, Friday, June 9th, 2023 at Glenhardie

By checking the consent box, I agree that:

1. I am the parent or legal guardian of the Child/Children named in this form above. I grant my Child(ren) permission to participate in the New Eagle Elementary School (“NEES”) 4th Grade Pool Party to be held at Glenhardie Country Club in June 2019 (the “Event”).

2. By consenting electronically to this Parent Permission and Liability Waiver (“Waiver”), I acknowledge that I am agreeing to not hold NEES, New Eagle Elementary PTO (the “PTO”), and/or the Tredyffrin-Easttown School District (“TESD”) liable for any injury or other damage related to or arising from my Child’s participation in the Event. This Waiver may only be completed by the Child’s parent/legal guardian.

3. I have indicated above the swim ability of each child in the form field(s) above. If I have indicated that my Child cannot swim independently, I agree that I or another parent/legal guardian will supervise the Child for the duration of the Event. I assume full responsibility and liability for the safety and well-being of my Child during the Event.

4. As parent/legal guardian of the Child, I am in agreement with all the following terms: I understand the inherent nature of the sport of swimming and the risk of bodily harm associated with my or my Child’s participation in the Event and assume all risks of participation. The Child(ren) is/are are in good health, and in proper physical condition to participate in this Event. I release, discharge, covenant not to sue, and agree to indemnify, save and hold harmless the PTO, NEES, and TESD from any and all liability claims, demands, losses or damages which may be suffered by the Child or the undersigned arising out of their participation in the Event, which was caused or alleged to be caused in whole or in part by any alleged negligence whether proven
or not of the PTO, NEES, or TESD, as well as their respective board members, parents, teachers, employees or any other related or associated members or persons. I further agree that if, despite this release, I, the Child, or anyone on the Child’s behalf makes a claim against the PTO, NEES and/or TESD, I will indemnify, save and hold harmless the PTO, NEES, and/or TESD from all claims, litigation expenses, attorney fees, loss liability, damage(s) or cost that the PTO, NEES, and/or TESD may incur as a result of any such claim(s).

5. By consenting electronically to this Waiver, I also give my full permission for first aid to be provided to the Child(ren) at the Event if determined necessary by any adult at the Event. I further authorize the transportation of the Child(ren) to a hospital if determined to be necessary by any adult at the Event.

Checking the consent box to these terms serves as my electronic signature and consent to all terms herein.
